Priority Health consistently surpasses national averages for managing chronic illness.
Priority Health offers proactive, ongoing help for members with chronic conditions, such as:
  • Asthma
  • Diabetes
  • Cardiovascular disease
  • Congestive heart failure
  • Childhood attention-deficit/hyperactivity disorder (ADHD)
  • Chronic obstructive pulmonary disease
  • Chronic pain

All members are eligible for this service, also known as disease management, when they enroll in a Priority Health plan. As soon as a member is identified as needing help, our in-house team takes action, providing educational tools, telephone support and face-to-face assistance. We also work with physicians to coordinate care.  The result? Members get the care they need, when they need it.
