It’s important to keep up on routine care, such as regular dentist appointments, so you can avoid potential problems down the road. PriorityMedicare D-SNP and PriorityMedicare D-SNP Advantage plans includes dental, vision and hearing coverage for you to take advantage of.

Preventive and comprehensive dental services through Delta Dental®

Find a dentist near you

Go to deltadentalmi.com/find-a-dentist. Choose Medicare Advantage PPO, put in your location and click Search. You will then see a list of available dentist offices near you to choose from and schedule an appointment. You can also call Delta Dental at 800.330.2732 (TTY 711). 

  • $0 for two exams and two cleanings per year (regular or periodontal maintenance) each year
  • $0 for one brush biopsy, one fluoride treatment and one set of bitewing x-rays each year.
  • $0 for periapical radiographs as needed
  • $0 for all other radiographs (full-mouth series or panoramic x-rays) every 24 months
  • $0 for simple and surgical extraction of teeth (once per tooth per lifetime)
  • $0 for two additional periodontal maintenance cleanings (four total each year)
  • $0 for non-surgical periodontal procedures (scaling and root planing)
  • $0 for crown repairs, once per tooth every 12 months
  • $0 for fillings (resin and amalgam on anterior teeth), once per tooth every two years
  • $0 for anesthesia, no limit when used during qualifying dental services
  • $0 for bridges and dentures (once every 60 months) and relines and repairs to bridges and dentures (once every 36 months, per appliance)
  • $2,500 annual maximum on all covered services for PriorityMedicare D-SNP
  • $4,000 annual maximum on all covered services for PriorityMedicare D-SNP Advantage

  • $0 for one retinal imaging per year
  • $200 for eyewear, such as glasses or contacts

Call TruHearing to speak to a hearing consultant at 833.714.5355 (TTY 711), Monday through Friday from 8 a.m. to 8 p.m. 

  • $0 routine hearing exam per year
  • One advanced-level hearing aid per ear, per year, with a 60-day trial period
  • $0 for all follow-up provider visits within the first year of hearing aid purchase
  • 80 batteries per hearing aid for non-rechargeable models
