Your plan includes a $0 Annual Wellness Visit each calendar year. This is a chance to meet with your physician to develop or update a personalized prevention plan based on your current health and risk factors.

Every Priority Health D-SNP member is assigned a care manager at no additional cost. Your care manager works closely with your primary care provider (PCP) to create a care plan that will help you manage your health.
They can help coordinate your care, make sure you have necessary medical supplies and work with you towards achieving your health and wellness goals. They’ll also support you after a hospital stay by assisting you in scheduling follow-up appointments, reviewing changes to your medications and more.
Regular communication with your care manager will happen over the phone.

The day your coverage begins, you can set up your account online, get your One Pass member code and get started right away.
With your plan, you receive a $70 monthly PriorityFlex** allowance to use toward over-the-counter (OTC) health and wellness items; healthy food, produce and meal delivery services; pest control; and select utility services like gas, water, sewer, septic, trash removal, electric, internet and phone.
You can shop in-store, online or by phone, and you can pay for utilities and pest control services as you would with any other Mastercard.
You'll receive a PriorityFlex card and OTC catalog in the mail. Your card will be pre-loaded with funds and ready to use on the day your plan begins.
Purchasing OTC Items

In your online member account, you'll be able to check your available balance, shop online, find participating stores, search eligible products, view your transaction history and more. The video below will show you how to set up and navigate your member account.
There are three ways you can shop for OTC items with your PriorityFlex benefit:
A PIN will be required to shop at Costco. No PIN is required when shopping at other retailers.
To set your pin:
1. Log into your member account and click "Manage card" on your account dashboard.
2. Select "Card management" on the left hand navigation, then click on "manage"
3. A pop-up window will appear. When prompted, select "set/reset your PIN"
Purchasing Healthy Food and Produce
Shop in-store: Shop at Meijer, Kroger, CVS, Walgreens, Walmart, and other participating store locations near you. You can search for retailers near you at any time by going to priorityhealth.com/shopOTC and using the "Find a Store" tool.
Purchasing Utilities, Pest Control Services or Meal Delivery
Pay for pest control services and select utilities with your PriorityFlex card as you would with any other Mastercard. You can place meal delivery orders through your OTC account at priorityhealth.com/shopOTC.

SafeRide provides no-cost transportation to and from medical appointments and other health-related locations. Get up to 30 one-way trips per year, within 100 miles with just a phone call. You may also submit for mileage reimbursement, up to 100 miles per one-way trip. Schedule a ride by calling 855.932.5418 (TTY 711) 6 a.m. to 6 p.m., Monday - Saturday.
