Annual Wellness Visit

Your plan includes a $0 Annual Wellness Visit each calendar year to meet with your physician to develop or update a personalized prevention plan based on your current health and risk factors.

One Pass®

One Pass can help you reach your fitness goals, while finding new passions along the way. Find a routine that's right for you whether you work out at home or at the gym.*

One Pass includes:

  • Access to the largest nationwide network of gyms, fitness locations and boutique studios
  • Live, digital fitness classes and on-demand workouts
  • CogniFit® online brain training made just for you to help improve your memory and focus
  • Groups, clubs and social events near you so that you can meet like-minded people
  • Meal delivery services with free shipping to make healthy eating easy**

Getting Started

The day your coverage begins, you can set up your account online, get your One Pass member code and get started right away.

  1. Create your account 
  2. Access your One Pass member code, search the network of gyms to find locations in your area, access digital classes, your CogniFit subscription and other member benefits.
  3. (Optional) Download the One Pass app and log in to access your account, programs and content from your mobile device.
  4. If you already belong to an in-network gym: After your coverage begins, stop by the front desk of your gym with your One Pass member code to make sure you are set up and ready to go.
    If you plan to join a participating gym or fitness center: Bring your One Pass member code with you and a staff member will help you set up your membership. If you forget your code, in-network gyms will be able to look it up for you.

CogniFit®

Train your brain with games to improve memory and focus. CogniFit supports more than 20 cognitive skills, including working memory, perception, attention, reasoning and coordination. Access CogniFit through your One Pass account.

Galleri®: Multi-cancer early detection test

The Galleri multi-cancer early detection test from GRAIL is a proactive blood test that screens for many deadly cancers before symptoms appear***. The Galleri multi-cancer early detection test is the first-of-its-kind:

  • A proactive blood test that screens for a "fingerprint" of 50+ cancers, including many with no recommended screening available today.
  • Galleri is a screening test and does not diagnose cancer, further testing is required to confirm cancer.

If a cancer signal is detected, Galleri predicts the tissue type or organ associated with the cancer signal to help healthcare providers guide the next steps.

Available to members who have one of the following: history of smoking, HPV diagnosis or elevated BMI.

Teladoc Health Mental Health

Teladoc Health Mental Health is a free mental wellness resource that offers support for stress, depression, sleep and more to help you live your happiest and healthiest life. 

Teladoc Health Mental Health empowers you with:

  • A personalized plan. Answer a series of questions and Teladoc Health will create a plan designed just for you.
  • Recommended digital content and resources. Explore self-guided activities and tools based on your goals and needs.
  • In-the-moment tools. Mindfulness and meditation techniques to boost your mood and help you focus on the present.
  • Dedicated support. Connect with a coach to encourage and support you through your journey.

Simply go to the Teladoc Health website or app, click "Create a new account" or "Set up your account" to get started.

ThriveFlex

With your plan, you receive a $60 quarterly ThriveFlex allowance to use toward over-the-counter (OTC) health and wellness products.

Get started with your online member account at priorityhealth.com/shopOTC.

In your online member account, you'll be able to check your available balance, shop online, find participating stores, search eligible products, view your transaction history and more. The video below will show you how to set up and navigate your member account.

There are three ways you can shop for OTC items with your ThriveFlex benefit:

  • Order online: View and purchase products online anytime at priorityhealth.com/shopOTC or through the OTC mobile app.
  • Shop in-store: Shop at Costco (PIN required at checkout), Meijer, Kroger, CVS, Walgreens, Walmart, and other participating store locations near you. You can search for retailers near you at any time by going to priorityhealth.com/shopOTC and using the "Find a Store" tool.
  • Call for delivery: Call 833.415.4380 (TTY 711) Sunday - Saturday 8 a.m. - 8 p.m. EST to place an order after reviewing the items in your OTC catalog. Have your selected product names, OTC card number, and shipping information available.

This allowance will be reloaded at the end of each quarter. Be sure to use your OTC allowance each quarter, your unused funds do not roll over.

You will also receive a $185 annual wellness allowance to use toward fitness equipment, exercise facilities and more.

  • To use your wellness allowance towards fitness facilities, simply go into a location and use your card as you would any other MasterCard.
  • Eligible fitness equipment includes items such as: dumbbells, pedalers, resistance bands, treadmills, ellipticals, stationary bikes, yoga mats, jump ropes, exercise balls, and foam rollers.
    These items can be purchased at participating retailers near you. For a full list of participating retailers, visit PriorityHealth.com/shopOTC or download the Priority Health OTC mobile app and use the Find A Store tool.
  • To use your wellness allowance toward nutrition support, you can use your ThriveFlex card to purchase memberships at Calm, Headspace, myFitness Pal, Noom and Weight Watchers to help you maintain and improve your overall well-being. Simply pay for these memberships the same way you would with any other MasterCard.

Excluded wellness items include:

  • Memberships at shooting ranges and archery.
  • Sporting good merchandise
  • Athletic clothing and shoes
  • Fitness equipment rentals
