Pay with my Social Security Check
You can have your premium payment deducted straight from your monthly Social Security Administration (SSA) check.
Important things to know:
-
The Social Security Administration will start deducting your premiums the first of the month after acceptance of your request.
-
You will need to pay any outstanding premiums prior to the month listed above by sending payment to:
Priority Health
3915 Momentum Place
Chicago, IL 60689-5339
Please include your payment coupon or write your subscriber ID on your check.
-
The first time SSA deducts from your Social Security check can be up to three months' premiums. This is based on the time it takes for them to process the request.
If for any reason your request is not accepted or is delayed longer than three months, Medicare will stop your request. Priority Health will notify you and bill you directly for your premiums.
