Priority Health chosen by state to provide new MI Coordinated Health dual eligible benefit program

(GRAND RAPIDS, Mich. — January 22, 2025) Priority Health, a nationally recognized nonprofit health plan serving more than 1.3 million members, has been awarded a contract to offer a new Medicare-Medicaid plan across all regions in Michigan’s Lower Peninsula.

The new health plan, MI Coordinated Health, was awarded by the Michigan Department of Health and Human Services (MDHHS) and will launch Jan. 1, 2026 in select counties before expanding statewide in 2027. It is a Highly Integrated Dual Eligible Special Needs Plan (HIDE-SNP) that will be available to the more than 300,000 individuals enrolled in both Medicare and Medicaid, who have full dual eligibility and are 21 years old or older.

MI Coordinated Health is the successor to the MI Health Link program, which ends Dec. 31, 2025. Current MI Health Link enrollees and all full dual eligible beneficiaries who are 21 and older, will have the option of moving into MI Coordinated Health with no break in coverage.  

A HIDE-SNP is a specific type of Medicare Advantage plan that is designed to meet the needs of those dually eligible for Medicare and Medicaid, and in Michigan it’s referred to as MI Coordinated Health. It will integrate long-term service and supports (LTSS), just like the MI Health Link program.

Contracts were awarded based on the state’s 10 Prosperity Regions. Priority Health was awarded the following regions, launching Jan. 1, 2026:
• Region 8 (Southwest Prosperity Region plus Barry County)
• Region 10 (Detroit Metro Prosperity Region – Macomb)
• Region 10 (Detroit Metro Prosperity Region, also referred to as Michigan HIDE-SNP Region 12 – Wayne)

Priority Health was also awarded the following regions, launching Jan. 1, 2027:
• Region 2 (Northwest Prosperity Region)
• Region 3 (Northeast Prosperity Region)
• Region 4 (West Michigan Prosperity Alliance, not including Barry County)
• Region 5 (East Central Michigan Prosperity Region)
• Region 6 (East Michigan Prosperity Region)
• Region 7 (South Central Prosperity Region)
• Region 9 (Southeast Prosperity Region
• Region 10 (Detroit Metro Prosperity Region also referred to as Michigan HIDE-SNP Region 11 – Oakland)

MI Coordinated Health is the result of information gathered from the MI Health Link program combined with a robust stakeholder feedback process dating back to 2022, which included surveys and feedback from beneficiaries, advocates, providers and health plans.
