Reminder: Authorizations for some varicose vein procedures will be required starting Jan. 1, 2025

As we shared in October, we'll require prior authorization following InterQual® criteria for certain varicose vein procedures effective Jan. 1, 2025, for commercial and Medicaid plans and in 2026 for Medicare plans.
