Narrow networks

We offer four narrow networks as part of our individual MyPriority℠ plan offerings. Our narrow networks are listed below.

With a narrow network plan, members receive reduced cost-sharing while receiving care in-network. If a member receives care outside of the network, the member is liable for all costs and care will not be covered.

We ask you, as providers, to assist patients by referring them within their network. Use Find a Doctor to see which providers are in your patient’s network. Request authorization for transplants and in instances where services are not available within the patient’s network.

MyPriority Corewell Health West Michigan network

Available to: Individuals under 65 who are members of individual MyPriority HMO narrow network plans

Identification: ID cards indicate "Corewell Health West Michigan Network."

Counties: Ottawa, Newaygo, Mecosta, Kent, Barry, and parts of Allegan County

In-network providers:

  • Hospitals: All Corewell Health hospitals in West Michigan
  • Physicians: Corewell Health Medical Group West, and physicians who denote a Corewell Health hospital in Grand Rapids as their primary hospital affiliation
  • Ancillary facilities: All those available in the Corewell Health West Michigan network; examples include Mary Free Bed Rehabilitation Hospital, Pine Rest Christian Mental Health Services and Forest View Hospital. Also includes all in-network pharmacies.

MyPriority Southeast Michigan network

Available to: Individuals under 65 who are members of individual MyPriority HMO narrow network plans

Identification: Member ID cards indicate "Southeast Michigan Network."

Counties: Macomb, Oakland and Wayne counties

Network providers: 

  • Corewell Health hospitals in Southeast Michigan, including Corewell Health Dearborn Hospital, Corewell Health Farmington Hills Hospital, Corewell Health Taylor Hospital, Corewell Health Trenton Hospital, Corewell Health Beaumont Troy Hospital, Corewell Health Wayne Hospital, Corewell Health Beaumont Royal Oak Hospital, and Corewell Health Beaumont Grosse Pointe Hospital
  • Trinity Health hospitals, including Trinity Health Chelsea Hospital, Trinity Health St. Joseph Mercy Ann Arbor Hospital, Trinity Health St. Joseph Mercy Livingston Hospital, Trinity Health St. Joseph Mercy Oakland Hospital, and Trinity Health St. Joseph Mercy Livonia Hospital
  • All physicians employed by Corewell Health Medical Group East in Southeast Michigan
  • Any individual community physician with admitting privileges at Corewell Health hospitals in Southeast Michigan who is listed in our Find a Doctor directory as in-network in the Priority Health Southeast Michigan Network
  • Any provider affiliated with Trinity Health East, including McAuley Health Partners, IHA OOMA, Oakland Physicians Network Services (OPNS), Oakland Southfield Physicians, Huron Valley Physician Association (HVPA) also known as Village MD, Livingston Physicians Organization (LPO) and Olympia
  • All ancillary providers and in-network pharmacies

MyPriority Bronson Healthcare Partners

Available to: Individuals under 65 who are members of individual MyPriority HMO narrow network plans

Identification: Member ID cards indicate "Bronson Healthcare Partners."

In-network providers:

  • Hospitals: Bronson hospitals (all campuses)
  • Physicians: Bronson Medical Group physicians, Bronson Healthcare hospital-based physicians, and physicians who denote a Bronson hospital (all campuses) as their primary hospital affiliation
  • Ancillary facilities: All ancillary facilities with a direct contract with Priority Health, except Borgess ancillary sites

MyPriority Trinity Health East network

Available to: Individuals under 65 who are members of individual MyPriority HMO narrow network plans

Identification: Member ID cards indicate "Trinity Health East Network"
