1 visit 3 -5 days after discharge

0-2 years


1 visit at 2, 4, 6, 9, 12, 15, 18 and 24 months

3-6 years


1 visit at 30 months and 1 visit every year for ages 3-6

7-10 years


1 visit every 1-2 years

11-18 years


1 visit every year
